Got Plans? Advance Care Planning Community Workshop – Trellis Supportive Care

October 10 @ 2:00 PM - 3:30 PM

Event Series Event Series (See All)
Free

We plan for college, marriage, a baby, and retirement, but we don’t prepare for the end of life.  Let’s change that!  Join us to discuss and learn more about Advance Directives and why it is important to choose someone who will speak for you when you cannot speak for yourself in a medical setting.  During the workshop, Jacob Gordon, Director of Community Programming with Trellis Supportive Care, will answer questions and guide participants through the Healthcare Power of Attorney and Living Will forms.

For those who want to complete the forms during our session, our staff notarize them for you at the end.  To register, contact Mr. Gordon by email at jgordon@trellissupport.org or by phone at 336-331-1341.

Advance Care Planning is a process where you reflect upon and plan for the care you would want if you are unable to make decisions for yourself.  Advance care planning helps your loved ones and healthcare providers honor your goals and wishes if you are unable to speak for yourself.  If you are ever in an accident, have surgery, or get a serious illness, you may need someone to advocate for your wishes.  Share this information with loved ones and put your wishes in writing.
