Join us to discuss and learn more about Advance Directives and why it is important to choose someone who will speak for you when you cannot speak for yourself in a medical setting. During the workshop, Jacob Gordon, Director of Community Programming with Trellis Supportive Care, will answer questions and guide participants through the Healthcare Power of Attorney and Living Will forms.

Advance Care Planning is a process where you reflect upon and plan for the care you would want if you are unable to make decisions for yourself. Advance care planning helps your loved ones and healthcare providers honor your goals and wishes if you are unable to speak for yourself. If you are ever in an accident, have surgery, or get a serious illness, you may need someone to advocate for your wishes. Share this information with loved ones and put your wishes in writing.
